Select a document. Click the document you want to download to open it in your browser. This option is in the upper-left side of the page. A drop-down menu will appear. If you're on a Mac, make sure you're clicking File in the web browser, not File in the upper-left corner of your Mac's screen.

Sorry about the issue you are facing. It is likely that your Google Form is shared with multiple people. If they too have configured the Email Notifications add-on, the emails will go out from their email account resulting in duplicate emails. You need to do this under the email account that is sending the duplicates. I have been using these to notify my team of reports they have submitted to keep track of their work. When the notifications are working it is amazing in allowing tracking and providing confidence that their work went through.

However they randomly turn themselves off. If I was a business that relied on this to provide my customers an automated response I would be fuming. LOVE this add on.
